Please be aware that you will be asked to complete a Screening Document prior to your appointment which will involve an e-signature. This form will cover information I am required to know before carrying out an appointment, such as allergies and ailments. Failure to complete and submit this form may result in your appointment being cancelled.
There will be no other changes to Body Smarts’ practice on this date.
